The effect of doula continuous care on decreasing caesarean rate and other pregnancy outcomes

Abstract

Objective To explore the effect of doula continuous care on reducing caesarean rate and other pregnancy outcomes.Methods This retrospective study was conducted in six hospitals, where doula continuous care was provided, located in adjacent Weihai and Yantai cities in Shandong province of China. Relevant data was collected from eligible pregnant women through a self-designed questionnaire. Pregnant women were divided into doula group or non-doula group according to whether they accepted doula continuous care during delivery. The statistical differences of related outcomes between two groups were examined using T-test and Chi-square test.Results A total of 1573 pregnant women (755 in doula group and 818 in non-doula) was included in the study. As compared with women in non-doula group, those women in doula group had lower rates of caesarean section (9.3% vs 4.0%) and epidural anesthesia use in the first labor stage (55.3% vs 40.5%), but higher rate of labor induction (15.3% vs 20.9%).Conclusion Doula continuous care could significantly decrease the rates of caesarean section and epidural anesthesia use. Professional doula continuous care should be promoted.
