Abstract:Objective To compare the results of two different ligation techniques, the traditional inguinal ligation of the internal spermatic veins and the high-position-tiny-incision microsurgery varicocelectomy, over semen quality, postoperative pregnancy rate and relapse of varicocele.Methods Postoperative semen quality, relapse rate and the spouses' pregnancy rate in 3 months and 6 months after surgery were compared between two randomly divided experimental groups of male varicocele patients: the high-position-tiny-incision microsurgery varicocelectomy group and the traditional inguinal ligation of the internal spermatic veins group, with 30 patients in each group.Results Three months after surgery, the sperm concentration of the traditional technique group showed a significant increase compared to preoperative parameters. Both sperm concentration and total vitality parameter had a significant increase in the microsurgical group after the operation. Sperm concentration of the microsurgical group was higher than that of the other group. Six month after surgery, sperm concentration and total vitality of both groups showed an increase, and the sperm concentration of the microsurgical group was significantly higher than that of the other group. Compared to the traditional technique group, microsurgery group had a higher postoperative pregnancy rate and a lower relapse rate.Conclusion Compared to the traditional inguinal ligation of the internal spermatic veins, the high-position-tiny-incision microsurgery varicocelectomy offers a better therapeutic effect to varicocele and should be considered a choice of treatment.
