Abstract:Objective To assess equity and inequity in obstetric resource allocation in some large and medium-sized cities for the purpose of providing information on policy-making in maternal and child health care.Methods Beijing, Shenzhen, Wuhan, and Chengdu were selected as the focused surveillance areas, and data in 2013 were analyzed. Lorenz curve, Gini coefficient, and Theil index were used to analyze and evaluate equity in obstetric resources.Results According to the number of pregnant women, the Gini coefficient of obstetricians, nurses, midwives, and maternity beds was 0.2213, 0.1927, 0.1695, and 0.1465, respectively, while the Gini coefficient was 0.3832, 0.4064, 0.3902, and 0.3338, respectively, when assessed by the size of the population. According to the size of service area, the Gini coefficient was 0.7043, 0.7324, 0.7322, and 0.6851, respectively. The contribution to the size of service area by the difference among cities was about 30%. This contribution within city was very low in Beijing and Shenzhen, but high in Wuhan and Chengdu, about 30% for both.Conclusion When assessed by the number of pregnant women and the size of the population, equity was observed in the number of obstetrician, the number of midwives, and the number of maternity beds. Some inequity was noticed in the number of nurses, and inequity was present in all of the aspects when assessed by the size of service area. The differences between the cities accounted for only about 30% of inequity, and the internal difference of Chengdu and Wuhan was the main causes of inequity when assessed by the size of the service area.
