The association between endometrial thickness and risk of pregnancy failure among women undergoing in vitro fertilization-embryo transfer

Abstract:Objective To assess the association between endometrial thickness (EMT) and the risks for pregnancy failure of women undergoing in vitro fertilization-embryo transfer (IVF-ET).Methods A prospective cohort study was conducted. Based on an ongoing IVF-ET cohort, the subjects who received fresh embryo transfer were recruited from October 2016 to May 2017 in the Reproductive Department of Yantai Yuhuangding Hospital Affiliated to Qingdao University, Shandong Province. A detailed questionnaire was used to collect demographic characteristics including age, body mass index (BMI), and smoking. The EMT of IVF-ET women on the day of human chorionic gonadotropin (hCG) injection was obtained from hospital clinical records. The trends of associations of EMT and risks of pregnancy failure of IVF-ET women was evaluated by generalized additive model (GAM). The modified Poisson regression model was used to assess the associations between EMT and risks of pregnancy failure of IVF-ET women.Results A total of 134 IVF-ET women who received fresh embryo transfer were included in the study. The biochemical pregnancy rate was 64.18% and the clinical pregnancy rate was 55.97%. The EMT on the day of hCG injection was 12 (10,12) mm. The results of GAM showed that the risks of pregnancy failure decreased with EMT. Modified Poisson regression models showed that the risk of biochemical pregnancy failure of IVF-ET women had an average decrease of 16% with the increase of 1 mm EMT (P=0.014), and the risk of clinical pregnancy failure of IVF-ET women decreased by 11% with every 1 mm increase in EMT (P=0.036).Conclusion The EMT on the day of hCG injection was associated with lower risks of IVF-ET pregnancy failure, indicating that a thick EMT is necessary to improve IVF-ET pregnancy outcomes. Accordingly, clinical treatments might be adopted to increase the EMT for IVF-ET women with thin EMT.
