Abstract:Objective To examine changes in nutrient composition in breast milk and to investigate feeding patterns of highly-educated women in different lactation period.Methods A total of 87 postnatal women with at least bachelor's degree were enrolled and followed-up. Breast milk samples in the 3-5 days, 7-10 days, (30±3)days, (90±3)days, and (180±3) days after delivery were collected, and nutrient contents were analyzed. Feeding options of participants were also investigated in these 5 lactating time points.Results Mature milk samples collected on the 30th, 90th, and 180th day had significantly lower content of lactose than colostrum and transitional milk. Lipid content in transitional milk and in 30-day mature milk was higher than colostrum. Colostrum and transitional milk had significantly higher protein content, compared with the rest mature milk samples. Total calories of transitional milk were significantly higher than those of colostrum and mature milk on 30th and 180th day. Minerals in colostrum and transitional milk were significantly higher than all three mature milk time points. Moisture contents in mature milk collected on the 90th day and the 180th day were significantly higher than that in the transitional milk. 68 of the 87 women chose breast milk feeding within 5 days after delivery, achieving an exclusive breastfeeding rate of 39.7%. On the 14th day after delivery, 72.5% lactating women whose breast milk sample were collected achieved exclusive breastfeeding. This rate increased to 85.1% on the 30th day postpartum. After 3 months, exclusive breastfeeding rate was 90.0%, and 81.2% of lactating women chose exclusive breastfeeding after 6 months.Conclusion Protein, lactose, and minerals in breast milk gradually decrease with the lactation time. Colostrum has lowest lipid and energy level but transitional milk has the highest value. Both lipid and energy content become stable in mature milk. As the secretion volume of breast milk was stable on the 14th day, highly-educated women preferred to choose exclusive breastfeeding, and exclusive breastfeeding rate increased gradually in the following time points.
