Relationship between neutrophil to lymphocyte ratio (NLR) in the first trimester and secretory Immunoglobin A (sIgA) in breast milk

Abstract:Objective To investigate whether the neutrophil-lymphocyte ratio (NLR) in the first trimester is associated with secretory immunoglobulin A (sIgA) in mature breast milk.Methods Subjects were recruited in Peking University Renmin Hospital from June 2018 to June 2019. Breast milk were collected on the day of outpatient and were stored at -80℃. Dietary frequency questionnaire was used to collect dietary information during lactation. Information on maternal demographic characteristics and blood routine examination in the first trimester was obtained from Nutritional Department. ELISA was used to determine SIgA concentration in breast milk. NLR, age, BMI, lactation stage, parity, preterm birth, mode of delivery, average daily protein intake, average daily energy intake and physical activity were included in the multivariate linear regression model.Results A total of 206 pregnant women were included in the analysis. The median and interquartile range [P25, P75] of breast milk days was 44.0[42.0, 49.0] d, and median NLR was 2.9 [2.2, 3.5], with range of 1.3-6.8. The average concentration of sIgA in breast milk was (1 798.1±213.0) μg/ml. SIgA was correlated with NLR, age, breastfeeding days, parity and energy-adjusted daily protein intake. The concentration of sIgA in breast milk increased by 32.6 μ g / ml for each unit increase of NLR. Sensitivity analysis showed that NLR was still positively correlated with SIgA concentration after removing top 5% neutrophils and lymphocytes.Conclusion NLR in first trimester is positively associated with sIgA in mature human milk.
