Median effective dose of dexmedetomidine on responses to extubation during recovery from general anesthesia in preeclampsia patients undergoing cesarean section

Abstract:Objective To determine the median effective dose (ED50) of dexmedetomidine inhibiting responses to extubation during recovery from general anesthesia in preeclampsia patients undergoing cesarean section.Methods This study included one hundred ASA physical status II or III patients with preeclampsia scheduled for cesarean section under general anesthesia. All patients were divided into 2 groups: mild preeclampsia (group I, n=50) and severe preeclampsia (group II, n=50). The initial dose of dexmedetomidine was 0.5 μg/kg and the ratio between the two successive doses was 1.2. The dose was increased/decreased in the next patient. Dexmedetomidine was infused intravenously over 15 min after fetal disengagement. The response to extubation was defined as positive when systolic blood pressure rose 25% above baseline values and maintained at this level for more than 1 min. The ED50 and 95% confidence interval (CI) were calculated by probit analysis.Results The ED50 (95% CI) of inhibiting responses to extubation was 0.5 μg/kg (0.4~0.5 μg/kg) in group I and 0.6 μg/kg (0.6~0.7 μg/kg) in group II.Conclusion The ED50 of dexmedetomidine inhibiting responses to extubation during recovery from general anesthesia was 0.5 μg/kg and 0.6 μg/kg, respectively, in mild and severe preeclampsia patients undergoing cesarean section.
